At PrevMed, we focus on heart attack, stroke, and cognitive decline. We serve patients who have already experienced an event as well as those who have not developed a diagnosis or event. Dr. Brewer is located in Lexington, KY and provides services via telemedicine or in person if you're in the Lexington area.
Patient-Directed Care - What is it? You’ll hear the term when you work with us. Basically, we set up systems for you to learn what you need - and why you need it. Then we set up systems for you to get what you need. That includes labs, imaging, and consultations with Drs. Brewer and Derrickson, and other providers. Don’t worry, we’ll teach you how. Instead of “giving you a fish, we’ll teach you how to fish”. That’s our methodology. We think you’ll love it. Most of our patients do. John Lorscheider is our patient Peer Counselor. He’s created many programs that will to teach you how to access and control your medical care.
RE: Prior Authorization for LABS, CIMT, and other diagnostic studies
This is the short story:
As an alternative for those who may be uninsured, or under insured, we can provide you with information on how to obtain our recommended labs and imaging via online Direct-to-Consumer lab providers, often at a reasonable cost to you. You will be required to pay them directly by credit card. You will then be provided requisitions from them that you will take to the service provider. Please note that these Direct-to-Consumer will not accept your insurance as payment, nor will they file your claims with your insurer.
The COURSES - We have a new digital course on Cardiovascular Inflammation which is now available for purchase. This course and future courses will prepare you for Patient-Directed Care. Future courses are currently in development for Insulin Resistance (PreDiabetes or Metabolic Syndrome), cIMT, Dietary Management, exercise and other key topics.
MoCA - The Montreal Cognitive Assessment is something I recommend for all patients. The #1 cause of Alzheimer’s Disease is CV Inflammation. Insulin Resistance is the major cause of CV Inflammation - and therefore cognitive decline. According to a recent study by UCLA, over half of us (30 years & older) have insulin resistance. By the time you’ve got noticeable cognitive decline, your biggest opportunity for prevention is gone. So, get tested now. Test - don’t guess. And, yes, you can reverse cognitive decline, especially in the early stages.

Includes a 30-45 minute session to collaboratively develop a health vision and specific actionable goals based on the individual's priorities for lifestyle changes with focus on health and wellness behaviors. Covered topics include: food and nourishment, exercise and movement, environment and fall prevention, sleep, and mind-body connection.
Includes both the Lifestyle and Wellness Coaching and a separate Montreal Cognitive Assessment (MoCA) in a 1 hour time period.
The MoCA assesses different cognitive domains including visual-spatial, executive function, attention, language, short term memory, and orientation.

Includes three follow up appointments for 30 minutes to discuss progress on lifestyle and wellness actionable goals and provision of resources to assist with new or existing goals.
